2018 NYS Title IV-E Plan Amendment (Submittal 18-06)
New York State Title IV-E Plan Amendment. Title IV-E of the Social Security Act provides federal funding for foster care, adoption assistance, and kinship guardianship assistance payments and administration that helps provide safe and stable out-of-home care for children until the children are safely returned home, placed permanently with adoptive families, or placed in other planned arrangements for permanency. This amendment addresses additional new requirements for states due to the Family First Prevention Services Act enacted in February 2018.

This ADM also introduces the Foster Care Placement Verification Form (OCFS-5184) for the purpose of providing such documentation, and provides instructions on when and how it must be completed.
